Н.Н. Блохина МЗ РФ с 2000 г. 31 пациент получил лечение с метастазами увеальной меланомы в печени. Из них: мужчины 13 (42 %), женщины – 18 (58 %). Средний возраст 45,1±13,2 лет, диапазон 23–71 лет. Объем поражения печени был до 25 % у 11 больных, до 50 % у 13, до 75 % у 7 больных. Всего было выполнено 62 ТАХЭ-вмешательства. В качестве эмболизирующей составляющей использовались: у 31 пациента – липиодол 10 мл в 53 случаях, микросферы – в 17 случаях (размеры частиц варьировались от 100 до 500 мкм), у 6 пациентов были использованы комбинации представленных эмболизирующих материалов. В качестве химиопрепарата выступал доксорубицин в дозировке 100 мг, или гемзар 1–2 г, или карбоплатин 450 мг.</p></sec><sec><title>Результаты</title><p>Результаты: В 100 % случаев ТАХЭ была технически успешной. Осложнений, связанных с техническим выполнением трансартериальной химиоэмболизации, не наблюдалось. Локальный ответ наблюдался у 21/31 (67,7 %) пациента с метастазами увеальной меланомы. Длительность наблюдения за пациентами после проведенной ТАХЭ была от 2 до 60 мес. Медиана наблюдения при этом составила 10 мес. Общая 1-летняя выживаемость составила 51,0 ± 9,5 %, 2-летняя 34,0 ± 15,2 %. Медиана выживаемости – 12,9 мес. 20 пациентов живы на момент исследования, 11 умерли от прогрессирования заболевания.</p></sec><sec><title>Выводы</title><p>Выводы: В нашем исследовании трансартериальная химиоэмболизация у больных с метастазами увеальной меланомы в печени зарекомендовала себя как эффективный и хорошо переносимый метод лечения. Объективный ответ или стабилизация процесса в печени наблюдалась у 68 % пациентов, причем медиана общей выживаемости сопоставима с таковой, представленной в литературных данных.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Purpose</title><p>Purpose: To evaluate the safety and efficacy of transarterial chemoembolization method (TACE) in the treatment of inoperable patients with uveal melanoma with isolated liver metastases with cured the primary focus.</p></sec><sec><title>Material and methods</title><p>Material and methods: In N.N. Blokhin NMRCO since 2000, 31 patients received treatment with metastatic uveal melanoma in the liver. Among them: men 13 (42 %), women 18 (58 %). The average age of 45.1 ± 13.2 years, range 23–71 years. Liver lesion volume was 25 % in patients 11 to 50 % in 13 to 75 % in 7 patients. Total 62 TACE intervention was performed. As used embolizing materials: 31 patients lipiodol 10 ml in 53 cases, microspheres – in 17 cases (particle sizes ranged from 100 to 500 microns), combinations presented embolizing materials have been used in 6 patients. As the chemotherapy Doxorubicin performed at a dosage of 100 mg, or Gemzar 1–2 g, or Carboplatin 450 mg.</p></sec><sec><title>Results</title><p>Results: 100 % of the TACE was technically successful. Complications related to the technical implementation not observed. Local response was observed in 21/31 (67.7 %) patients with metastatic uveal melanoma. Duration of monitoring of patients after TACE was held from 2 to 60 months. Overall 1-year survival in this group of patients was 51.0 ± 9.5 %. 2-year 34.0 ± 15.2 %. The median survival – 12.9 month. 20 patients were alive at the time of the study, 11 died from progression.</p></sec><sec><title>Conclusions</title><p>Conclusions: In our study transarterial chemoembolization in patients with metastatic uveal melanoma in the liver has established itself as an effective and well-tolerated treatment. Objective response or stabilization process in the liver was observed in 68 % of patients.
