ПКР традиционно считается радиорезистентным к конвенциальной лучевой терапии, в связи с чем она не применялась при лечении больных первичным ПКР. Наиболее актуальное направление, развивающееся в последние годы, было посвящено поиску рациональных режимов лучевой терапии, позволяющих преодолеть радиорезистентность ПКР.</p><p>Предоперационная и послеоперационная лучевая терапия: Попытки предоперационной и послеоперационной лучевой терапии в проведенных исследованиях не позволяют судить о преимуществе такого подхода, так как не дают преимуществ в выживаемости этих пациентов.</p><p>Лучевая терапия первичного неоперабельного ПКР: Для преодоления радиорезистентности ПКР к стандартной лучевой терапии требуется подведение больших доз за фракцию. Благодаря стереотаксической лучевой терапии появилась возможность высокоточного поражения мишени с применением высоких доз излучения и уменьшения лучевой нагрузки на прилегающие к опухоли здоровые ткани и критические органы.</p></sec><sec><title>Заключение</title><p>Заключение: Хотя ПКР относится к радиорезистентным опухолям, в последние годы обозначились новые перспективные направления в лучевой терапии. Лучевая терапия обеспечивает хороший локальный контроль опухоли при подведении больших доз за фракцию. Благодаря появлению новых технологий лучевой терапии, к которым относится стереотаксическая лучевая терапия, появилась возможность точной доставки доз ионизирующего излучения в опухоль без опасности повреждения соседних тканей и органов.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction: Renal cell cancer accounts for 2 % of all cancers. The gold standard for managing patients with no evidence of distant metastasis renal cell cancer remains is complete surgical resection. Renal cell cancer is radioresistant tumor for conventional radiation therapy. Although renal cell carcinoma is related to radioresistant tumors, in recent years new promising directions in radiation therapy have become apparent.</p><p>Preoperative and postoperative irradiation: The clinical data investigating preoperative radiotherapy failed to reveal benefited from this methods. The role of routine postoperative radiotherapy in the management of renal cell cancer is not established in patients with localized disease after complete surgical resection.</p><p>Primary irradiation for localized renal cell cancer:</p><p>To overcome the radioresistance of renal cell carcinoma, the use of modified radiation therapy regimens with high doses per fraction is justified. А new technologies of radiation therapy, which include stereotactic radiation therapy (SRT) allows to accurately deliver doses of ionizing radiation to a tumor, without the risk of damage to neighboring tissues and organs.</p></sec><sec><title>Conclusion</title><p>Conclusion: Recent data showing that with the use of high-precision methods, such as SRT, unresectable local renal cell carcinoma can successfully be treated with durable local control and low toxicity.
