Рак молочной железы (РМЖ) является самым распространенным онкологическим заболеванием среди женского населения во всем мире. На его долю приходится более 16 % всех случаев злокачественных новообразований у женщин. Техническое развитие лучевых методов диагностики опухолей молочных желез привело к увеличению доли вы­являемых при обследовании непальпируемых образований, в связи с чем появилась необходимость усовершенствования методик получения биоптатов опухолей с использованием средств навигационного контроля. Также важным аспектом успешности лечения и выбора терапевтической стратегии является информация о метаболическом статусе опухоли, кото­рый играет роль в патоморфологическом ответе на проводимую терапию. Одними из общепризнанных методик оценки местного метаболизма опухолей являются оптические технологии — флуоресцентная спектроскопия и спектроскопия диффузного отражения.</p></sec><sec><title>Цель</title><p>Цель. Оценка эффективности применения мультимодальной оптической диагностики в определении метаболического статуса опухолей молочных желез у пациенток с подозрением на РМЖ in vivo в ходе выполнения биопсии.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Проведены обследования 21 пациентки с новообразованиями молочных желез, которым помимо стандартных методик обследования проводилась оценка местного метаболизма опухолей методами флуоресцентной спектроскопии и спектроскопии диффузного отражения с помощью специально разработанного устройства с тонкои­гольным оптоволоконным зондом.</p></sec><sec><title>Результаты</title><p>Результаты. Установлено, что злокачественные опухоли молочных желез обладают более низкими значениями тканевой сатурации (6,6 % [5,7-24,8 %], Me [Q1-Q3]) по сравнению как с доброкачественными опухолями (66,3 %[43,7-74,8 %]), так и c окружающей здоровой тканью (79,0 % [77,6-84,4 %]). Также обнаружено, что злокачественные опухоли имеют более высокие значения интенсивности флуоресценции на длине волны возбуждения 365 нм (6157 отн.ед. [2188-8814 отн.ед.]) по сравнению с доброкачественными (2962 отн.ед. [1924-4301 отн.ед.]), что может быть связано с накоплением кофермента НАДН или изменениями коллагенового матрикса.</p></sec><sec><title>Заключение</title><p>Заключение. Предложенный метод оценки местного метаболизма опухолей молочных желез может применяться для улучшения результатов прицельных биопсий под ультразвуковым контролем и оценки лечебного патоморфоза РМЖ. С целью оценки эффективности данного метода диагностики требуется продолжение исследования с набором большего количества пациенток для получения статистически достоверных данных.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Breast cancer (BC) is the most common malignant tumor in women worldwide. It amounts more than 16 % of all cases of malignant neoplasms in women. Modern advances in radiological methods of breast tumors detection significantly increased the number of non-palpable neoplasms revealed during examinations, while further progress requires the development of improved minimally invasive techniques for obtaining biopsy material from breast tumors using tools of navigational controls. An approach with high potential for direct evaluation of local tumor metabolism consists in complementary use of fluorescence spectroscopy and diffuse reflectance spectroscopy.</p></sec><sec><title>Purpose</title><p>Purpose: Evaluation of the effectiveness of a new complementary approach for the diagnosis of the metabolic status of breast tumors in patients suspicious for breast cancer.</p></sec><sec><title>Material and methods</title><p>Material and methods. This article presents the results of the examination of 21 patients with breast neoplasms who, in addition to standard examination methods, including ultrasound, X-ray mammography, puncture biopsy and trepanobiopsy, were subject to evaluation of local tumor metabolism with complementary employment of fluorescence spectroscopy and diffuse reflectance spectroscopy .</p></sec><sec><title>Results</title><p>Results. It has been established that the malignant breast tumors have lower saturation values compared with both benign tumors and the surrounding healthy tissue: StO2(BC) = 6,6 % [5,7-24,8 %]; 79,0 % [77,6-84,4 %], StO2(fibroadenoma) = 66,3 %[43,7-74,8 %]. It is also found that the malignant tumors have higher values of NADPH compared with the benign tumors (INAD(P) H(BC) = 6157 a.u. [2188-8814 a.u.]; INADH (fibroadenoma) = 2962 a.u. [1924-4301 a.u. ]).</p></sec><sec><title>Conclusion</title><p>Conclusion. The new method of the diagnosis of the local breast tumor metabolism can be further used to improve the results of targeted biopsies under ultrasound control and to assess the therapeutic pathomorphosis of BC.
