Необходим поиск факторов прогноза, которые помогут индивидуализировать лечение и обеспечат наилучшие отдаленные результаты.</p></sec><sec><title>Цель</title><p>Цель: Оценить отдаленные показатели общей и безрецидивной выживаемости в зависимости от очередности локальных методов лечения, а также определить факторы, которые оказывают наибольшее влияние на отдаленные результаты лечения пациентов с отеком кожи при РМЖ.</p></sec><sec><title>Материал и методы</title><p>Материал и методы: Представлены результаты однофакторного анализа и отдаленные результаты лечения 182 пациенток с сохраняющимся отеком кожи после проведения системного лечения, разделенных на три группы в зависимости от объемов и очередности локальных методов лечения. В I группу (91 пациент) вошли больные, которым проводилась системная лекарственная терапия, операция и дистанционная лучевая терапия (ДЛТ). Во II группе (41 пациент) больные получали только лекарственную терапию и ДЛТ. В III группе (50 пациентов) проводились системная противоопухолевая терапия и ДЛТ, а далее было выполнено удаление первичной опухоли.</p></sec><sec><title>Результаты</title><p>Результаты: Результаты однофакторного анализа продемонстрировали, что статистически значимо на показатели безрецидивной выживаемости влияют: прогрессирование отека кожи молочной железы после проведенной системной терапии (р = 0,05), длительный промежуток между системной терапией и началом локальной терапии (более 9 нед, р = 0,028), большое количество пораженных лимфатических узлов (р = 0,01), отсутствие лечебного патоморфоза в опухоли (р = 0,013) и в лимфатических узлах (р = 0,014). При однофакторном анализе результатов общей выживаемости наиболее значимыми признаками оказались: возраст пациентов (р = 0,034), степень лечебного патоморфоза в лимфатических узлах (р = 0,05), количество пораженных лимфоузлов (р = 0,021).</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction: Neoadjuvant treatment may not provide complete clinical response at the first stage in some patients with skin edema associated with breast cancer. This complicates selection of the subsequent local treatment method. We need to identify predictors that may help personalize the treatment and ensure the best possible long-term outcomes.</p></sec><sec><title>Purpose</title><p>Purpose: To evaluate long-term outcomes (overall and relapse-free survival) in relation to the sequence of local treatment modalities and identify factors that have the greatest impact on the long-term outcomes of patients with skin edema associated with breast cancer.</p></sec><sec><title>Material and methods</title><p>Material and methods: The publication presents single-factor analysis and long-terms outcomes of 182 patients with persistent skin edema after systemic treatment. The patients were divided into three groups depending on the volume and sequence of local treatment modalities. Group I (n = 91 patient) included patients who received systemic treatment, surgery, and external beam radiotherapy (EBRT). Group II (n = 41) included patients who received only the systemic treatment and EBRT. Group III (n = 50) included patients who received systemic treatment and EBRT followed by resection of the primary tumor.</p></sec><sec><title>Results</title><p>Results: The single-factor analysis showed that progression of breast skin edema after systemic treatment (р = 0.05), long break between the systemic treatment and start of the local treatment (more than 9 weeks, р = 0.028), large number of diseased nodes (р = 0.01), non-pCR (р = 0.013) and lymph nodes (р = 0.014) has a statistically significant effect on the relapse-free survival.
