В терапии применяют иммунохимиотерапию, лучевое и хирургическое лечение, но стандартов нет. В диагностике этого заболевания эндоскопический метод исследования является обязательным, но применение совмещенной позитронно-эмиссионной и компьютерной томографии с 18F-ФДГ (ПЭТ/КТ) на разных этапах наблюдения и лечения, несмотря на наличие в российских рекомендациях указаний на их применение, мало освещено в литературе.Цель — оценка чувствительности, специфичности и диагностической точности ПЭТ/КТ с 18F-ФДГ на этапах наблюдения и иммунохимиолучевого лечения больных экстранодальной ДВКЛ желудка.Материал и методы: Изучены непосредственные и отдаленные результаты лечения 23 первичных больных экстранодальной ДВКЛ желудка, получивших и закончивших иммунохимиотерапию по схеме R-СНОР и лучевое лечение в РНЦРХТ в период 2010–2018 гг. На всех этапах наблюдения проводили ПЭТ/КТ-исследование и фиброгастродуоденоскопию (ФГДС) у всех пациентов.Результаты: Общая и безрецидивная 5-летняя выживаемость всей группы больных экстранодальной ДВКЛ желудка составила 91,5  % и 100  % соответственно. Диагностическая точность ПЭТ/КТ с 18F-ФДГ у больных экстранодальной ДВКЛ желудка до лечения составила 87 %, после иммунохимиотерапии — 91,3 %, после лучевой терапии — 100 %.Заключение: Продемонстрирована высокая эффективность иммунополихимиолучевой терапии у больных экстранодальной ДВКЛ желудка. Показана целесообразность комплексного обследования у больных экстранодальной ДВКЛ желудка на всех этапах наблюдения и лечения для выработки долгосрочного прогноза.</p></abstract><trans-abstract xml:lang="en"><p>Introduction: Gastric extranodal diffuse large B-cell lymphoma (DLBCL) is one of the most common types of lymphoma. The management of patients with DLBCL of s tomach includes immunochemotherapy, radiotherapy and surgery, but there is no standard approach. Endoscopy as a part of diagnostic workup is obligatory, but using positron emission tomography/computed tomography (PET/CT) with 18F-fluorodeoxyglucose (18F-FDG) is not obligatory at different stages of observation and treatment.Purpose: To evaluate the diagnostic precision of PET/CT with 18F-FDG in both assesement and treatment stages for patients with extranodal DLBCL of stomach. Materials and methods. In this study we describe early and late therapeutic outcomes of 23 patients with primary extranodal DLBCL of stomach, treated in our center between 2010 and 2018, who received R-CHOP immunochemotherapy regimen.
