Гибридная томогафическая технология, объединяющая возможности пространственного представления накопления радиофармпрепарата и анатомической его привязки, позволяет не только точно выявлять пораженную костную структуру, но и по рентгенологической семиотике определять природу изменений. При обследовании онкологических больных применение ОФЭКТ/КТ позволяет не только выявить опухолевую патологию в костях, но и исключить ошибки и дополнительные обследования.</p></sec><sec><title>Цель</title><p>Цель: Продемонстрировать наиболее частые заболевания и состояния, симулирующие при радионуклидном обследовании опухолевую патологию в костях.</p></sec><sec><title>Материал и методы</title><p>Материал и методы: Проанализированы результаты исследований 81 больного с различными опухолевыми заболеваниями с 2015 по 2020 гг. Сканирование всего тела проводили в режиме всего тела с остеотропным РФП 99mТсфосфотех через 3 ч после в/в введения на томографе Symbia E, T2 (Siemens, Германия). Затем выполняли ОФЭКТ/КТ.</p></sec><sec><title>Результаты</title><p>Результаты: Наиболее частыми причинами появления очагов повышенного накопления РФП стали: реактивное усиление метаболизма в результате дополнительной минеральной пластики, в том числе деформирующие остеоартрозы, краевые остеофиты, посттравматические и послеоперационные изменения; переломы (на фоне остеопороза и «стресс»-переломы); воспалительные процессы различной этиологии; очаги, обусловленные доброкачественными новообразованиями костей и дисплазиями. Сочетание высокочувствительной, но низкоспецифической сцинтиграфии с высокоспецифичной компьютерной томографией делает ее особенно полезной в анатомически сложных областях.</p></sec><sec><title>Выводы</title><p>Выводы: Гибридизированная система ОФЭКТ/КТ позволяет выявлять прямую зависимость анатомо-структурных нарушений с изменением тканевого метаболизма при повреждениях костей различного генеза и минимизирует ряд неоднозначных выводов. Применение ОФЭКТ/КТ в онкологической практике значительно облегчает дифференциально-диагностический процесс для врача-радиолога и сокращает время обследования для пациента. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance: Radionuclide bone scan signs of lesions are not specific and require radiological identification. Hybrid tomographic technology is a combination of spatial distribution of radiopharmaceuticals and its anatomical binding. Thus, makes it possible to accurately identify the affected bone structure and also determine the nature of this changes (radiological anatomy). In cancer patients, SPECT/CT method allows to visualize tumor pathology in the bones, but also keeps to a minimum errors and other excessive examinations.</p></sec><sec><title>Purpose</title><p>Purpose: To demonstrate the most frequent diseases and conditions that simulate tumor lesions in bones on bone scan.</p></sec><sec><title>Material and methods</title><p>Material and methods: We analyzed the results of examinations of 81 patients with various tumor diseases (2015–2020). Bone scan was performed in the whole body 3 hours later, after intravenous administration of 99mTc-phosphotech on a Symbia E, T2 (Siemens, Germany). SPECT/CT was performed after WB Bone Scan.</p></sec><sec><title>Results</title><p>Results: The most frequent reasons for the focal increased uptake of radiopharmaceutical were: reactive increase in metabolism and as a result additional mineral repair: deforming osteoarthritis, osteophytes; post‑traumatic and postoperative changes; fractures (osteoporosis and stress fractures); inflammatory processes; focal uptake due to the benign neoplasms of bones and dysplasia. Combination of highly sensitive but non-specific scintigraphy with highly specific computed tomography makes it especially useful in anatomically difficult areas.</p></sec><sec><title>Conclusion</title><p>Conclusion: SPECT/CT reveals a direct pater of anatomical and structural abnormalities with changes in bone tissue metabolism in case of different injuries and minimizes a number of ambiguous conclusions.
