У большинства больных РП пищевода диагностируется на III–IV стадиях заболевания. В настоящее время стандартом лечения неоперабельных больных РП является одновременная химиолучевая терапия. Применение брахитерапии в комплексном лечении больных РП позволяет увеличить дозу в мишени без увеличения риска осложнений, связанных с эскалацией дозы облучения.</p></sec><sec><title>Материал и методы</title><p>Материал и методы: В исследование вошло 20 пациентов неоперабельным РП. Всем больным было отказано в хирургическом лечении в связи наличием тяжелых сопутствующих заболеваний. У 12 (60 %) больных выявлена Т3N1–3М0 G1–3 стадия, у 8 — Т1–2N0–1М0G2–3. Возраст пациентов варьировал от 51 до 89 лет. Исходный соматический статус по шкале ECOG 1 имел место у 5 (25 %) и ECOG 2 — у 15 (75 %) больных. Дисфагия 1-ой степени отмечалась у 5 (25 %) больных, 2-ой степени — у 10 (50 %) и 3–4-ой степени — у 5 (25 %). У всех пациентов диагностирован плоскоклеточный РП. У большинства больных опухоль локализовалась в средней трети пищевода (70 %). Наибольшая протяженность опухоли составила 10 см и отмечалась только у одного больного. На первом этапе пациентам выполнялась дистанционная лучевая терапия (ЛТ) на пищевод и регионарные зоны РОД 2 Гр, СОД 46 Гр с одномоментной химиотерапией (ХТ). После стихания лучевых реакций пациентам была проведена в брахитерапия (БТ) высокой мощности дозы: РОД 5 Гр, 2 фракции с интервалом 6–7 дней. Доза за курс лечения составила 62,8 Гр (EQD2, α/β = 10 Гр).</p></sec><sec><title>Результаты</title><p>Результаты: Все больные получали поддерживающую терапию и нутритивную поддержку до ХЛТ, в процессе и после лечения. Все больные завершили запланированную радикальную программу лечения. Ни у одного больного не было отмечено гематологической токсичности 3–4-ой степени. Средняя продолжительность наблюдения за больными составила 12 мес. Эзофагиты 2-ой степени наблюдались у 40 % больных, эзофагиты 1-ой степени — у 60 % больных. Степень эзофагита оценивали по шкале токсичности CTCAE v4.0 (Common Terminology Criteria for Adverse Events). У всех больных клинические проявления эзофагита купировались с использованием противовоспалительных, обволакивающих и обезболивающих препаратов. У 4 (20 %) больных развилась рубцовая стриктура пищевода и у одного больного сформировался пищеводно-бронхиальный свищ через 3 мес после завершения лечения. Полная регрессия опухоли и регионарных лимфатических узлов отмечена у 2 (10 %) больных, частичная регрессия — у 16 (80 %). У 2 (10 %) пациентов зарегистрирована стабилизация процесса. После завершения лечения у всех больных выявлена положительная динамика со стороны общего состояния здоровья и эмоционального функционирования.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background: The incidence of esophageal cancer is increasing. In this connection, the search for the most effective methods of treating this disease continues. In most patients, esophageal cancer is diagnosed at III– IV stages of the disease. Currently, the standard of treatment for inoperable patients is chemo radiotherapy. Brachytherapy is one of the perspective methods of radiation therapy. Using of brachytherapy in the complex treatment of patients with esophageal cancer makes possible to increase the radiation dose without increasing the risk of complications after treatment.</p></sec><sec><title>Methods</title><p>Methods: The study included 20 patients with inoperable esophageal cancer (most of them with III stages, 60 %). The age of the patients is from 51 to 89 years, ECOG performance status ½ (25 %/75 %), dysphagia score 1/2/3–4 (25 %,50 %, 25 %). All patients were diagnosed with esophageal squamous cell carcinoma. Most patients were with tumor in the middle third of the esophagus (70 %). The greatest length of the tumor was 10 cm. On the first stage of treatment patients underwent external-beam radiation therapy 46 Gy in 2 Gy fractions with simultaneous chemotherapy. After that, brachytherapy was performed (single dose of 5 Gy, 2 fractions with an interval of 6–7 days, 62,8 Gy (EQD2, α/β = 10 Gy).</p></sec><sec><title>Results</title><p>Results: Patients received therapy support for cardiovascular and lung diseases and nutritional support before, during and following chemoradiotherapy. There were no patients with hematological toxicity grade 3–4. Esophagitis grade 2 was noted in 40 % of patients, grade 1 in 60 % of patients. Esophagitis resolved on its own 2 weeks after completion of therapy. Complete tumor regression was observed in 10 % of patients, partial regression in 80 % of patients, stabilization of tumor process was observed in 10 % of patients. All patients have a poor QOL due to physical and psychological disorders.
