Разработана оригинальная методика индивидуальной объемно-контролируемой транзиторно-гипоксической селективной внутриартериальной химиоинфузии, которая сравнивалась со стандартной внутривенной химиотерапией по схеме PF при лечении рецидивов плоскоклеточного рака головы и шеи.</p><sec><title>Материал и методы</title><p>Материал и методы: В исследование включено 73 пациента с рецидивами плоскоклеточного рака головы и шеи после завершенного химиолучевого лечения до СОД 68-70 Гр со статусом ECOG ≥ 2. В 1-ю группу включили 39 пациентов, пролеченных с использованием оригинальной методики селективной внутриартериальной химиоинфузии. Во 2-ю группу (контрольную) включили 34 пациента, пролеченных с помощью стандартной химиотерапии с внутривенным путем введения по схеме PF.</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение: Технический успех селективных внутриартериальных вмешательств у всех пациентов 1-й группы составил 100 %. Различий в частоте и выраженности нежелательных реакций, обусловленных химиотерапией, в обеих группах получено не было. Частота объективных ответов составила 30,8 % в 1-й группе и 11,8 % во 2-й группе (р &lt; 0,01). Стабилизация отмечена у 59 % больных в 1-й группе и 52,9 % во 2-й группе (р &gt; 0,05). Частота прогрессирования составила 10,3 % и 35,3 % в 1-й и 2-й группе соответственно (р &lt; 0,01). Медиана выживаемости пациентов 1-й группы составила 325 сут, что почти в 2 раза выше, чем во 2-й группе — 173 сут.</p></sec><sec><title>Заключение</title><p>Заключение: Таким образом, разработанная методика внутриартериальной селективной объемно-контролируемой транзиторно-гипоксической химиоинфузии может являться методом выбора при лечении рецидива плоскоклеточного рака головы и шеи после завершенной лучевой терапии у ослабленных пациентов.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction: The squamous cell carcinoma of the head and neck treatment remains an urgent and still unresolved problem of modern oncology. We have developed an original method of individualized volume-controlled transient hypoxic selective intra-arterial chemo-infusion which was compared with standard intravenous chemotherapy according to the PF scheme for the treatment of recurrent squamous cell carcinoma of the head and neck.</p></sec><sec><title>Material and methods</title><p>Material and methods: Consecutively 73 patients with the ECOG ≥ 2 status and relapses of squamous cell carcinoma of the head and neck after previous completed chemo-radiotherapy up to total dose 68-70 Gy were included in this study. In the first group consecutively 39 patients were treated by the original technique of selective intra-arterial chemoinfusion. In the second group (control group) the 34 patients were treated by standard chemotherapy by PF scheme with intravenous administration.</p></sec><sec><title>Results and discussion</title><p>Results and discussion: The technical success of selective intra-arterial interventions in all patients in group 1 was 100 %. There were no differences in the frequency and severity of adverse reactions due to chemotherapy in both groups. The frequency of objective responses was 30.8 % in group 1 and 11.8 % in group 2, respectively (p &lt; 0.01). Stabilization was observed in 59 % in group 1 and 52.9 % in group 2, respectively (p &gt; 0.05). The rate of progression was 10.3 % and 35.3 % in group 1 and group 2, respectively (p &lt; 0.01).
