Глебовская В.В., Ткачев С.И., Борисова Т.Н., Алиева С.Б., Романов Д.С., Тимошкина Е.В., Мамедли З.З., Гордеев С.С., Бесова Н.С., 2020</copyright-statement><copyright-year>2020</copyright-year><copyright-holder xml:lang="ru">Глебовская В.В., Ткачев С.И., Борисова Т.Н., Алиева С.Б., Романов Д.С., Тимошкина Е.В., Мамедли З.З., Гордеев С.С., Бесова Н.С.</copyright-holder><copyright-holder xml:lang="en">Glebovskaya V.V., Tkachev S.I., Borisova T.N., Alieva S.B., Romanov D.S., Timoshkina E.V., Mamedli Z.Z., Gordeev S.S., Besova N.S.</copyright-holder><license xml:lang="ru" license-type="creative-commons-attribution" xlink:href="https://creativecommons.org/licenses/by/4.0/" xlink:type="simple"><license-p>Данная работа распространяется под лицензией Creative Commons Attribution 4.0.</license-p></license><license xml:lang="en" license-type="creative-commons-attribution" xlink:href="https://creativecommons.org/licenses/by/4.0/" xlink:type="simple"><license-p>This work is licensed under a Creative Commons Attribution 4.0 License.</license-p></license></permissions><self-uri xlink:href="https://www.oncoradjournal.ru/jour/article/view/117">https://www.oncoradjournal.ru/jour/article/view/117</self-uri><abstract><p>Увеличение числа излеченных больных плоскоклеточным раком анального канала и качество их жизни напрямую связано с совершенствованием технического оснащения радиотерапевтических отделений онкологических клиник.</p><p>Более точное определение опухолевого процесса путем использования современных диагностических методов, создающих возможность максимального воздействия на опухоль и выявленные пораженные лимфоузлы с ограничением дозы на критические органы.</p><p>За 2000-2015 гг. закончили лечение 301 больной плоскоклеточным раком анального канала в стадии T1-4N0-3M0-1, получавшие комплексное лечение с выполнением лучевой терапии в конвенциальном, конформном режиме, включая применение современных технологий — лучевую терапию с модуляцией интенсивности — IMRT и объемно-модулированную лучевую терапию арками — VMAT, в суммарной очаговой дозе 50-60 Гр.</p><p>Применение IMRT/VMAT в сравнении с конформной лучевой терапией позволило улучшить: непосредственные результаты: полная регрессия опухоли — 67,5 % vs 55,6 % (p = 0,071) и трехлетние отдаленные результаты лечения — общая выживаемость — 92,9 % vs 88,2 % (p = 0,05) соответственно, с тенденцией достоверности локорегионарного контроля — 89,9 % vs 81,6 % (p = 0,179).</p><p>Применение лучевой терапии в ее современном исполнении, с использованием современных методов визуализации позволило достичь высоких непосредственных и отдаленных онкологических результатов с определением границ опухолевого поражения и подведения планируемой дозы к первичной опухоли и пораженным лимфоузлам.</p></abstract><trans-abstract xml:lang="en"><p>The increase in the number of cured patients with squamous cell anal carcinoma and the quality of life is directly related to the improvement of radiotherapy departments technical equipment in oncological clinics.</p><p>Main goals are more accurate determination of the tumor volume by using modern diagnostic methods, and creating the possibility of maximum impact on the tumor and identified affected lymph nodes while limiting dose to organs at risk.</p><p>During the period from 2000 to 2015, 301 patients with squamous cell anal carcinoma, stage T1-4N0-3M0-1, were complex treated by radiation therapy with 2DRT, 3DCRT, or IMRT/VMAT in a total dose of 50-60 Gy.</p><p>The use of IMRT / VMAT in comparison with conformal radiation therapy allowed to improve: immediate results (complete tumor regression) 67.5 vs 55.6 % (p = 0.071) and 3-year long-term treatment results (overall survival) 92.9 vs 88.2 % (p = 0.05), respectively, with a tendency for the reliability of local-regional control 89.9 vs 81.6 % (p = 0.179).</p><p>The use of radiation therapy in its modern version, using modern imaging methods allowed to achieve high immediate and long-term oncological results with the definition of the boundaries of the tumor lesion and summing the planned dose to the primary tumor and the affected lymph nodes.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>плоскоклеточный рак</kwd><kwd>анальный канал</kwd><kwd>лучевая терапия с модуляцией интенсивности (IMRT)</kwd><kwd>объемно-модулированная лучевая терапия арками — VMAT</kwd><kwd>магнитно-резонансная томография</kwd></kwd-group><kwd-group xml:lang="en"><kwd>squamous cell carcinoma</kwd><kwd>anal canal</kwd><kwd>intensity-modulated radiation therapy (IMRT)</kwd><kwd>volume modulated arc therapy (VMAT)</kwd><kwd>magnetic resonance imaging (MRI)</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Johnson LG, Madeleine MM, Newcomer LM, Schwartz SM, Daling JR.
